The 2009 Site Assessor Checklist is available
AIHA Laboratory Accreditation Programs, LLC is committed to making the Site Assessor Checklist available to its accredited laboratories or other parties who already legally own a copy of the ISO 17025:2005 standard. To obtain a PDF version of the Site Assessor Checklist, you will need to send us proof of purchase of the ISO/IEC 17025:2005 standard (either a receipt or a signed affidavit, which can be found in the on-line Accreditation Application).

The AIHA Laboratory Accreditation Programs, LLC fee schedule consists of the AIHA Laboratory Accreditation Programs, LLC Enrollment forms and fees.
Note: Accreditation policies and other documentation included on this website reflect the operations of AIHA Laboratory Accreditation Programs, LLC but may refer to the American Industrial Hygiene Association or AIHA since AIHA-LAP, LLC programs were once operated by AIHA. These documents may also include AIHA accreditation program logos.
