The 2010 Site Assessor Checklist is available
AIHA Laboratory Accreditation Programs, LLC is committed to making the Site Assessor Checklist available to its accredited laboratories or other parties who already legally own a copy of the ISO 17025:2005 standard. To obtain the Site Assessor Checklist, you will need to send us proof of purchase of the ISO/IEC 17025:2005 standard (either a receipt or a signed affidavit, which can be found in the on-line Accreditation Application). Please note the new Accreditation Application instruction Section 1a): “Prior to submission of this application, the laboratory must submit Form 10 and/or proof of purchase of the ISO/IEC 17025:2005 standard and request a copy of the current AIHA-LAP, LLC Site Assessment Checklist.” This Checklist is now to be included as Attachment 7A2 of the application.
