File Upload

For your convenience, this form supports multiple file uploads. However, a single zipped file is preferred when submitting multiple files at once.

• Use the Choose File button to add files; hold Ctrl to select multiple files at a time.
• Click the Submit button to begin the upload.
  
• You will see a confirmation message when the upload is complete.

• When uploading your responses, the file name should not contain any special characters (including &, #, $, %, {, }, and ~).
• Upload size is limited to 60 MB.
• For larger submissions, we recommend that you compress them into a single .zip file, or break your submission into multiple smaller files that should be uploaded separately.
• If your upload is successful you will receive a confirmation page. If you do not receive a confirmation page, we did not receive your submission.
